At this time, only RETAKE applications are being accepted.  The 2024 window to apply for Initial or MOC certification closed on June 1, 2024 and will reopen in 2025 

We invite you to apply then. 

If applying for your 2024 RETAKE assessment, please use your Survey Monkey User ID and password that you created for your TAGME assessment.

There is a $150 retake fee.  Payment is required at the time of your retake application submission through Stripe with credit or debit card only.

Retakes must be completed by December 20, 2024
